With the exception of the Firefighter Physical, the references below are pulled from the CBJ website, which should be considered the primary reference for employee benefits. This page aims to underscore that information by highlighting aspects most relevant to us as members.

NFPA 1582 physical form adapted by Dr. Quigley for CCF/R employees. To obtain a firefighter physical, take this printed form to your primary care physician and have them follow it to complete.
Any costs not covered by insurance should be forwarded on to CCF/R Admin Office. The form SAYS hazmat at the top, but this is for all members, not just HazMat team. -

Open enrollment for employee benefits is May 13-31, 2024.
Under the Health/Dental/Vision tab, find the CBJ Plan Comparison 2023-2024 under Benefit Plan Comparisons to understand the differences between the High Deductible Health Plan, Economy and Standard plans. This matrix changed significantly for 2023-2024, make sure you know which plan is best for you and your family.

The CBJ has contracted ComPsych® to provide employees and their families with support, resources, and information on a confidential basis—at no charge. Through this benefit employees and all household members (regardless of their benefit status) are eligible to receive on demand telephone counseling, as well as up to 6 free sessions per issue with a local counselor.
To get started, visit the ComPsych® website, or to speak with a ComPsych representative by calling 1-800-295-9059 (TDD: 1-800-697-0353). When you call or set up your online account use the following information:

For more urgent situations, you can call and speak to a counselor on the phone immediately, 24/7.
If you wish to work with a local counselor for your 6 free sessions per issue, when you call you can either ask if a specific counselor you have in mind is contracted with them, describe your situation and receive a list of recommended counselors based on the issue, or ask for the full list of contracted counselors and choose from it.

Health Yourself Screening Form
This is the form you fill out annually to capture the health insurance discount of $50/pay period. If you are under the Economy Plan, this makes your plan free. You must earn 100 points and turn it in to Jess Brown, due May 31.
Comprehensive Blood Draws
These are available for benefit- eligible employees as well as spouses on our insurance plan. Tests include a chemistry & lipid panel, CBC and vitamin D tests. New to this year are HbA1C, TSH, and ApoB tests.
